Welcome to the student forms library, forms can be submitted online, via email to regdept@gvsu.edu, or in person at the Registrar's Office in Allendale (STU 150) or the Pew Campus (DEV 115C).

Online submission of forms requires students to login with their G number and PIN. We request that all printed forms please be completed in ink.
